Acne, is a common skin issue which is known to start in teens and go on into adulthood. It can go beyond menopause as well due to strong genetic and lifestyle components.

Causes of Acne

Sebaceous gland hyperactivity and inflammation are the two primary causes.

This has a lot to do with

  1. Diet (dairy, refined sugar, fried food and gluten intake),
  2. Habits (smoking and alcohol consumption especially with sugary mixtures in cocktails and wheat beers which are high caloric load and inflammatory properties)
  3. Sleep pattern ( irregular or reduced sleep hours especially in people with night shifts or in frequent flyers)
  4. Sedentary lifestyle.

In women it’s a bit more complex. The above causes can cause imbalance in 6 hormones- thyroid, cortisol, insulin, prolactin, 17 hydroxy progesterone and testosterone. The common condition being PCOS with highest incidence of about 29-35% last recorded in 2024 September in India. Jawline acne (acne in the lower part of the face) is the diagnostic point in hormonal acne. This can occur along with other findings like unwanted facial hair, patterned hair loss, etc.

In men however, there are primarily 2 hormones responsible for acne insulin and cortisol.

The variants

The more severe variants of acne include cyst formation and infection along with follicle involvement in underarms, inner thighs etc is called hidradenitis suppurativa.

The out of the box variants are acne fulminans, acne conglobata, recurrent folliculitis, dissecting folliculitis and cellulitis of scalp acne keloidalis nuchae.

Acne Scar Treatment

How to combat this naturally

The milder forms usually improve with diet and lifestyle modification.

  • Sleep – a good 7-8 hour sleep working around the circadian rhythm can moderate your cortisol levels.
  • Diet – staying away from sugary high carb diet
  • Exercise – a combination of cardio, weight training and yoga
  • Diet – including a lot of veggies and fruits with good fibre content is something you can easily do.

Stress management- sleep and exercise contribute a lot in this aspect. Pranayama and breathing right is the key.

Medical management

  • Inflammatory Acne – cystic acne needs a course of antibiotics to remove all the bacteria
  • Retinoids – for comedogenic variant we use these magic molecules to improve cellular turnover and reduce the tendency to scar.
  • Antioxidants – to help recover and reduce recurrence.

Identification of hormonal imbalance and correction.

Our in-clinic treatments

  1. Chemical peels- they help open the clogged hair follicle units by loosening the skin cells and improving cell turnover. Agents like salicylic acid, azelaic acid when used can reduce the sebaceous activity (oil production).

  2. Hollywood (Carbon LASER) peel- infusing carbon into the sebaceous glands and breaking down these particles helps open up the blocked units. Qswitch NdYAG laser also helps in fading out the hyperpigmentation associated with acne.

  3. Gold toning is very useful in inflammatory acne where there’s a lot of redness and pus formation. This works by primarily reducing the inflammation and blood vessel enlargement.
